Title:COMPARISON OF THE INTERNATIONAL HEADACHE SOCIETY CRITERIA AND THE 3-QUESTION HEADACHE SCREEN IN THE DIAGNOSIS OF MIGRAINE IN NIGERIANS

Content:Objectives: Migraine is a chronic disabling disorder that is under-diagnosed while the available preventive and abortive therapies are poorly utilized. A simple screening tool will result in rapid diagnosis of migraine especially in developing countries where the population is high and the doctor to patient ratio is low. The aim of this study was to compare the International Headache Society (IHS) criteria with the 3-question headache screen in the assessment of migraine in Nigerians.

Materials and Methods: 1513 students of a Nigerian university were screened for migraine with the IHS criteria. Those who satisfied the IHS criteria for migraine (145) were further requested to answer the following questions: [1]. Do you have recurrent headaches that interfere with work, family, or social function? [2]. Do your headaches last at least 4 hours? [3]. Have you had new or different headaches in the past 6 months? Diagnosis of migraine was made if the responses to the first 2 questions were in the affirmative and the response to the last one was no. The measures of agreement between the IHS criteria and the 3-question screen were determined.

Results: 86.9% of those who satisfied the IHS criteria screened positive for migraine on the use of the 3-question screen. There was a good agreement between the IHS criteria and the 3-question headache criteria (k =0.68, p<0.001). The sensitivity was 66.2% while the specificity was 97.8%. Positive predictive value was 76.2% while the negative predictive value was 96.5%.

Conclusion: There was a good agreement between the 3-question headache screen and the IHS criteria. It is thus recommended for rapid diagnosis of migraine.
